There's a lack of clarity about how HIV services are going to be planned and commissioned. A lot of smaller HIV charities are really struggling because they are not able to plan around the upheaval.

I manage a team which helps people with HIV. Many of the people I work with come from difficult backgrounds – they certainly couldn't afford to pay for their care.

The NHS is always undergoing reorganisation – but you are reorganising the same work rather than reconceptualising the way you do the work. It lurches from one crisis to the next and never takes a long-term view.
